Purpose
The purpose of this grant is to continue a successful transitions of care learning collaborative in which Northern California hospitals and their outpatient partners (physician practices, home health agencies, skilled nursing facilities, and others) share best practices with the common goals of improving the quality of transition from hospital to home and reducing unnecessary hospital readmissions.
