The process of returning home or transitioning to another care setting after a hospital stay can be challenging for patients and providers. Patients may have difficulty understanding their discharge instructions, receive conflicting information from various health care providers or experience serious medication errors, all of which can cause complications and return visits to the hospital.

With Moore Foundation support, the Health Research & Educational Trust of the American Hospital Association is creating a first-of-its-kind online community to strengthen the field of care transitions. The community, a real-time platform called Huddle for Care, will feature web-based content, live interaction and a listserv community to give users the opportunity to learn from and connect with others working to improve care transitions. The goal is to accelerate the spread of best practices and lessons learned to advance the field.
