The San Francisco Health Plan is a licensed community health plan created by the City and County of San Francisco to provide high quality medical care to the largest number of low-income San Francisco residents possible, while supporting San Francisco’s public and community-minded doctors, clinics and hospitals.

The San Francisco Health Plan provides affordable health care coverage to over 140,000 low and moderate-income families in San Francisco. The plan includes medical, dental and vision insurance. Members can choose from over 3,400 primary care providers and specialists, nine hospitals and over 200 pharmacies.

The health plan seeks to educate, inspire and assist members in leading healthy lifestyles. It maintains strong, collaborative relationships between members, community-based organizations and health care providers throughout San Francisco, and recognizes the cultural and linguistic diversity of its residents. The health plan leads with innovation, continually creating new ways to make health care more accessible and affordable, and fosters a team-oriented environment based on respect that supports personal and professional integrity.
