Based in San Francisco, On Lok Lifeways allows seniors to continue to live at home, supported by a dedicated, closely coordinated team of health care and social service professionals. The organization maintains and develops health care models for the well-being and dignity of the elderly and chronically ill through education, advocacy, innovation in services and financing, while advocating for quality and affordable care.
The nonprofit organization was formed in the early 1970s by a group of citizens concerned about the plight of elders and the lack of long-term options in the community. On Lok serves seniors in the community through several programs including: On Lok Lifeways, a comprehensive health plan that provides long-term care for eligible seniors living in the Bay Area; 30th Street Senior Center, the largest multi-purpose senior center in San Francisco offering a broad range of programs including a senior center, nutrition and congregate meals, home delivered meals and bilingual case management; and On Lok Intergenerational Program, which brings together On Lok Lifeways members and children from neighboring childcare centers and elementary schools.
