In support of creating a Readmissions Learning Community in which hospitals and their outpatient provider partners confirm and share evidence-based practices shown to reduce hospital readmissions. The outputs of the grant include participation by at least 50% of the 5-county San Francisco Bay Area hospitals, and the collaborative development of detailed plans to implement evidence-based practices, each designed to reduce 30- and 90-day hospital readmission rates by at least 30% by December 31, 2013.
