To implement and evaluate a transitional care program, in partnership with Palo Alto Medical Foundation and Peninsula Family Services, aimed at reducing hospital readmissions for patients age 65 and older. The outcome of this grant is to achieve a sustained reduction in both 30- and 90-day readmission rates for patients age 65 and older by at least 30% as compared to a 2011 baseline.
