Sutter Health System is using this grant to implement and evaluate a Continuum of Care model for congestive heart failure patients in two Sutter Health affiliates in the San Francisco Bay Area. Through the grant, Sutter Health will implement the Continuum of Care model in the Alta Bates and Summit campuses of the Alta Bates Summit Medical Center. By providing better coordinated and higher quality of care during and following hospitalization, the project is anticipated to result in improved patient outcomes and lower rates of hospital readmission for advanced heart failure patients.
