Hospital Council of Northern and Central California is using this grant to organize the Bay Area Patient Safety Collaborative into its next phase. The Bay Area Patient Safety Collaborative supports adult acute care hospitals in working together to improve hospital care processes in the San Francisco Bay Area. The grant supports hospital convening, quality education and on-site support to hospitals participating in the collaborative and working on improvements in patient outcomes. The grant outcome is an active Bay Area Patient Safety Collaborative, with a significant number of hospitals participating and working together on improving outcomes in their hospitals.
