For the development, implementation and evaluation of a new transitional care program at San Francisco General Hospital (SFGH), with the goal of improving outcomes for 65 year old patients through a better designed transition from hospital to home. By providing better coordinated and higher quality of care during and following hospitalization, the project is anticipated to result in lower rates of hospital readmission for elderly patients discharged from SFGH. The impact of the transitional care program supported by this grant will be documented through an evaluation and cost-benefit analysis which will be disseminated through publication and/or presentation at a national meeting.
